Special Needs Assessment and Plan
For clients with developmental, mental health or physical disabilities, we focus our attention on the programs that are available to support them to live with dignity and as much independence as can safely be provided. We look at their cognitive abilities and connect them with local mental health or intellectual or developmental disability professionals. We assess the safety of housing and transportation relative to any physical or mental limitations.
Financially we assess eligibility for Medicaid Waiver programs, Supplemental Security Income (SSI), Social Security Disability Insurance (SSDI) and SNAP for food assistance.
The assessment may reveal the need for strategies to protect against personal abuse and financial exploitation.
P. Harrison Group also does an educational assessment to explore support for special education programs (for example PPCD-12, or 504), Head Start, or Vocational or Trade School training.

Comprehensive Care Management for Children
We are the helping hand for families that need it the most. We help parents and caregivers manage the many challenges they face each day—from finding the right specialist for their child, to coordinating ARD services with schools, arranging for transportation to securing medical insurance.

Geriatric Assessment and Plan
P. Harrison Group will do a thorough assessment of cognitive, physical, social, and emotional abilities, as well as the availability of family members to help. We look at the safety of the home and determine the optimal setting for continued dignity and independence. A financial and legal review completes the picture so we can create a plan tailored to the individual’s needs.
The plan might include periodic wellness checks, coordination of home care, bill management, medical advocacy or end-of-life planning. It may be that a move to an assisted living or group home is the more appropriate setting.
A thorough assessment and plan can result in a benefits analysis to identify eligibility for state and federal programs and allow us to structure enrollment so that financial distributions do not jeopardize benefits. From Medicaid Waiver programs to SSI, SSDI and SNAP, we explore all options available and enroll the client in those that apply. By matching services to needs, our clients receive the support they deserve while assets remain protected.

Medical Advocacy
The health care system in our country is confusing. Some would even say broken. To get needed care, one truly needs a personal guide and advocate who can assure that needs are being addressed thoroughly and in a manner consistent with each individual’s goals of care.
Whether it’s accompanying the client to doctor visits, encouraging the client to seek needed care, or making sure they follow through with recommended therapies, P. Harrison Group is there beside them through the entire process. We coordinate care between providers and keep you abreast of health changes as they arise.

Hospital (or Rehab) to Home
A discharge is usually good news. Health is returning! But it also brings up questions of much needed support to be sure a still-fragile patient has what they need to recover quickly and completely at home.
P. Harrison Group staff can assist with translating discharge instructions into daily routines for medicine and exercise. Our care managers can coordinate services from many important providers such as home care, physical therapists, speech therapists, wound-care specialists. We also arrange follow-up visits with the client’s primary care providers and medical specialists. We don’t want our clients joining the 20% of patients who get readmitted due to inadequate support at home.

Solo Agers
No spouse? No children? Or at least none that are available to help? Solo agers are particularly vulnerable to unnecessary crises. Without adult children nearby to notice cognitive and functional changes—or predation by scammers and con artists,—solo agers are at risk for significant problems that could be prevented with early detection and regular check ins.
Let P. Harrison Group conduct an assessment to be sure that all the bases are covered: in-home care or appropriate placement, meal prep, medication assistance, transportation, home safety to reduce the chance of falls, and social activities to reduce isolation (and the health challenges that come with it). With regular check-ins we can monitor change and avert a crisis that would otherwise unfold without trained eyes and ears on the ground.

Wellness checks
Maybe the person you care for needs periodic phone calls to be sure all is well. For others, an in-person visit is more important. The care managers at P. Harrison Group are trained to ask questions and assess the situation to look for changes in physical or mental health, signs of abuse or financial exploitation, hazards in the household, medication misuse and myriad other points of potential concern.
If there are paid caregivers, our staff can monitor their performance to verify that the appropriate care is being given and matches the care billed for.
You receive regular updates about client wellness, and of course immediate communication if a problem is identified.

Bill management
Are shut off notices being sent to the house? Unexpected credit card purchases showing up on the statement? Paying bills and managing accounts are a significant part of independence. Plus, assets need to be protected. But in many situations—for example dementia or developmental disabilities—the ability to track expenses with consistency is just too much to handle.
P. Harrison Group can assume the tasks of daily money management, making sure utilities, house payments, insurance, taxes and other necessities are paid when they are due. Protected spending accounts can be arranged so client spending does not jeopardize assets nor eligibility for benefit programs.
We also monitor bank accounts for signs of financial exploitation and can arrange periodic check-ins to look for signs of undue influence.

Custodian Real Estate, Housing & Relocation
Everyone wants to live in their own home. But sometimes help is needed to make that a safe option.
P. Harrison Group can support independence by managing the responsibilities of home ownership. For instance, we make sure Home Owner Association fees, taxes and insurance get paid.
If a move is needed, we can assist with downsizing and relocation. We can also help to sell the house and have funds transferred in a way that protects assets in the trust and does not jeopardize benefits programs.

End-of-life Planning
From advance directives to burial plans, P. Harrison Group staff are trained to address the many decisions that must be made ahead of time so our clients have the care they want and are supported through a gentle death. For those who lack an advance directive, we refer to local estate planning attorneys to be sure this important paperwork is in order.
As health challenges arise, we counsel our clients about the different options available and when they are most relevant. We can describe the difference between palliative care and hospice, and generate a referral for the program that seems most in line with client values.
We can also help arrange for pre-need burial plans so the clients wishes are addressed and in place when the time arrives.
